Job Summary And Qualifications

Job Overview:

  • Demonstrates advanced clinical skills and ability to perform first evaluations and level-of-care determinations.
  • Coordinates with the treatment team to determine safe discharge plan, or inpatient transfer.
  • Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
  • Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
  • Completes advanced assessments and all other documentation promptly and in compliance with requirements, as appropriate to scope.
  • Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge. Provides guidance on complex issues.
  • Conducts post-discharge follow-up as indicated.
  • Evaluates re-admissions for gaps in discharge planning processes. Formulates a plan and implements new strategies.
  • Other duties as assigned.

What qualifications you will need:

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • Crisis Prevention Intervention must be obtained within 30 days of employment start date
  • Masters Degree- Required.
  • Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC), or Psychologist
