About Us

The Mālama Kidney Center was created to address the concerns, issues, and healthcare burden associated with Chronic Kidney Disease (CKD). The number of patients with CKD in Hawai'i continues to rise and is compounded by the shortage of Nephrologists to care for these patients. Mālama Kidney Center serves as an extension of Nephrologists' practice to provide education, support, and care coordination for patients with CKD.

Position Summary

The Care Manager plays a vital role in coordinating care, supporting patient self-management, and connecting patients to resources throughout their CKD journey. This role is responsible for conducting assessments, developing individualized care plans, and ensuring patients receive the education, support, and services needed to manage their condition. The ideal candidate has excellent customer service and communication skills and is proactive, detail-oriented, and passionate about delivering holistic, patient-centered care.

Key Responsibilities

  • Conduct comprehensive assessments of patients with CKD, including medical, psychosocial, and environmental needs.
  • Develop, implement, and monitor individualized care plans in collaboration with the patient, Nephrologist, PCP, Dietician, Social Worker, and other care team members.
  • Educate patients and families about CKD progression, treatment options, lifestyle modifications, medications, and preventive care.
  • Coordinate referrals to specialists, support services, and community resources.
  • Monitor patient progress, identify barriers to care, and adjust care plans as needed.
  • Track key performance indicators and patient outcomes; document activities in the electronic health record (EHR) in a timely and accurate manner.
  • Facilitate care transitions (e.g., hospitalization, dialysis initiation, transplant evaluation) to ensure continuity of care and reduce avoidable complications.
  • Advocate for patient needs while supporting adherence to evidence-based clinical guidelines.
  • Assesses physical and mental wellness and develops plans to improve overall health.
  • Actively and compassionately listen to patient concerns and provides counseling and/or intervention as appropriate.
  • Maintains patient confidentiality in accordance with federal, state, and local laws and regulations.
  • Establishes and maintains a positive relationship with area hospitals, agencies, vendors, and the community.
  • Performs other related duties as directed or required.

Qualifications

Required:

  • Bachelor’s Degree in Social Work or other related healthcare field
  • 2+ years of experience in case management, chronic disease care, or care coordination
  • Excellent communication and interpersonal skills
  • Ability to thrive in a team-based environment
  • Strong organizational skills and attention to detail
  • Comfortable working in a fast-paced, team-based environment

Preferred:

  • 1+ year of experience in a nephrology clinic, primary care, or managed care setting
  • Familiarity with chronic kidney disease

Job Type: Full-time

Pay: $45,000.00 - $60,000.00 per year
