Social Worker II - Palliative Care Department

Responsibilities:

Job Summary: Social Workers are responsible for providing necessary support for each patient referred to their discipline according to health status, diagnosis, and patient and family specific needs. Hospital social workers advocate and help patients understand and navigate complex systems. With compassion and cultural awareness, they empower individuals to make informed decisions during challenging times.

  • Collaboratively works with interdisciplinary teams to ensure completion of patient specific care plan and post-acute needs.
  • Effectively collaborates with community resources to align patients with available resources. Assists with referrals for community-based resources.
  • Completes required governmental assessments and level of care documents.
  • Provides counseling services to patients and families when appropriate.
  • Assists with various populations across the organization to ensure proper resources and support including but not limited to APS, CPS, DSS, disability based on identified patient safety and needs. Assists indigent population with needed resources to aid in prevention of readmissions and support quality in patient outcomes.
  • Consistently handles customer complaints promptly and effectively when problems encountered. Utilizes proper escalation procedures when appropriate.
  • Performs all other duties as assigned.

Work Schedule: 80 Hours Biweekly. Full Time

Qualifications:

1- 3 years related experience, computer literacy, excellent communication and writing skills.

Licensed Master Social Work with the SC licensing board.
