The program emphasizes coordination, communication and evidence-based care, resulting in an improved patient experience and outcome.A typical day will include: Working with a set of clinics, providers, and nurse care managers to coordinate and provide integrated care to treat the physical, mental, spiritual, and social needs of our patients.

Helps patients identify barriers and behaviors that lead to poor outcomes
Identifies goals and develops strategies or behavioral modifications to achieve goals
Assesses patients in the Emergency Department
Provides transitional care services by making referrals for patients moving from the ED and Primary Care into therapy

This position is: Full-time, Mon-Fri, 8am-4:30pm.Job Requirements:

Education: MSW or Master's in Counseling from an accredited institution
Experience: 1 year of experience preferred
Skills: Written and oral communication skills, time management, detail-oriented
Licensure/Certification: LMSW, LCSW, or LPC required
Physical: Sitting for prolonged periods of time; ability to lift up to 20 lbs. on an infrequent basis
