- Helps patients identify barriers and behaviors that lead to poor outcomes
- Identifies goals and develops strategies or behavioral modifications to achieve goals
- Assesses patients in the Emergency Department
- Provides transitional care services by making referrals for patients moving from the ED and Primary Care into therapy
- Education: MSW or Master's in Counseling from an accredited institution
- Experience: 1 year of experience preferred
- Skills: Written and oral communication skills, time management, detail-oriented
- Licensure/Certification: LMSW, LCSW, or LPC required
- Physical: Sitting for prolonged periods of time; ability to lift up to 20 lbs. on an infrequent basis
