Position Accountability
This position is responsible for the planning, implementation, and/or coordination of all treatment services delivered to clients that are assigned. This includes ensuring that established procedures are followed and that clinical standards are set for treatment planning, the delivery of individual case management services, group and psychoeducational didactics, and discharge planning and referral services are met. Significant creativity and innovation are necessary in order for clients to be successfully engaged, motivated, and supported in their efforts to achieve treatment plan objectives. Collaboration and coordination with both internal and external resources are crucial to successful client outcomes.
Tasks and Responsibilities
1. Responsible for service coordination which establishes a framework of action to enable the client to achieve specified goals.
2. Complete necessary administrative procedures at admission to caseload.
3. Establish accurate treatment and recovery expectations with the client including but not limited to:
o The nature of services Program goals
o Program procedures
o Rules regarding client conduct
o Schedule of treatment activities
o Factors affecting duration of care
o Client's rights and responsibilities
4. Reassess client's needs monthly, or more frequently when significant changes have occurred.
5. Provides required contacts, testing results, and documentation for each treatment service provided.
6. Successful collaboration with clients and significant others.
7. Engagement of client in Real House Inc. programs.
8. Responsible for the collaborative process in which the counselor and the client develop a written document that identifies important treatment goals, describes measurable, time sensitive action steps toward achieving those goals with expected outcomes and reflects an agreement between counselor and client.
9. Use of relevant assessment information to guide the treatment planning process.
10. Examine treatment options in collaboration with the client and significant others (when appropriate).
11. Formulate mutually agreed upon and measurable treatment goals and objectives.
12. Identify appropriate strategies for each treatment goal.
13. Develop with the client a mutually acceptable treatment plan and method for monitoring and evaluating progress.
14. Coordinate treatment activities and community resources in a manner consistent with the client's diagnosis and existing placement criteria.
15. Reassess the treatment plan monthly or more frequently when indicated by changing circumstances.
16. Establish a helping relationship with the client characterized by warmth, respect, genuineness, concreteness, and empathy.
17. Facilitate the client's engagement in the treatment and recovery process.
18. Promote client's knowledge, skills, and attitudes that contribute to a positive change in behaviors.
19. Encourage and reinforce client actions determined to be beneficial in progressing toward treatment goals.
20. Work appropriately with client to recognize and discourage all behaviors inconsistent with progress toward treatment goals.
21. Adapt counseling strategies to the individual characteristics of the client, including but not limited to disability, gender, sexual orientation, developmental level, culture, ethnicity, age, and health status.
22. Make constructive therapeutic responses when the client's behavior is inconsistent with stated recovery goals.
23. All treatment plans, progress notes, group notes, and any other documentation is completed according to policy, is legible, and reflects client participation in treatment.
24. All reports and/or documents requested by management are accurate and timely.
25. Ensures efficient programming occurs through volunteering coverage as needed.
26. Anticipates the needs of the clients and the program in order to ensure effective programming.
27. Responsible for adhering to all applicable Federal and State laws, the professional code of ethics prescribed by the Division of Consumer Affairs Licensure Board of New Jersey; the policies and procedures detailed in the Real House Inc.'s Policy and Procedure Manual, Code of Ethics statement and the instruction provided by supervision to include alternative duties. This includes the responsibility to make no statement or take any action that would knowingly threaten the best interests of the agency, other staff members, and/or of clients in our care. The holder of this position is also responsible for undertaking and sustaining a commitment to job related professional growth.
Qualifications
Education:
- Master’s degree in human services, social work, or related field
Experience:
- Minimum of 2 years experience in the field of Mental Health Treatment Services, co-occurring treatment experience preferred
Requirements:
- LSW or LAC (with preferred background in substance use)
- Able to communicate effectively (both written and oral skills)
- Ability to work independently
- Ability to work under pressure
- Familiarity with health and safety issues
- Two years of Sobriety prior to application (if applicable).
