Job Summary:
The Health Homes Care Manager Supervisor is responsible for the supervision of assigned Care Managers. This position requires carrying a reduced caseload while being accountable for ensuring adherence to the regulations indicated through the Department of Health and the Health Homes of Upstate NY (HHUNY) program.
Responsibilities:
- Provide direct care management services while maintaining accurate and verifiable documentation of all services rendered.
- Complete comprehensive and detailed assessments, reviews, crisis plans, and care plans as requested by the Department of Health.
- Oversee implementation of care plans to the satisfaction of the client served.
- Provide a core service for each client monthly as identified in the care plan to ensure the client’s goals are addressed.
- Oversee Health Homes Care Managers, as assigned by the Director of Health Homes.
- Assist Director with oversight of operations and personnel when necessary.
- Participate in mandatory and optional trainings as applicable.
- Ensure all documentation is completed within the timeframes mandated by the NYD Department of Health.
- Provide on-going training for Health Homes Care Managers to provide education on policies and procedures for the Health Homes programs, in addition to those of CCOR.
- Ensure Health Homes Care managers obtain required training from Health Homes to meet the eligibility required to provide services as required by the Department of Health.
- Complete audits of assessments, care planning, and care management documentation.
- Ensure Health Homes policies and procedures are maintained and compliant with NYS DOH regulations.
- Responsible for the growth of Health Homes services within the agency.

Pay: $25 per hour
Position Requirements:
- A Bachelor’s Degree in any of the following: Child & Family Studies, Community Mental Health, Counseling, Education, Nursing, Occupational Therapy, Physical Therapy, Psychology, Recreation, Recreational Therapy, Rehabilitation, Social Work, Sociology, or Speech & Hearing; OR NYS licensure and current registration as a Registered Nurse and a Bachelor’s Degree; OR a Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnosis; OR a Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
- Two years’ experience (Master’s Degree in a related field may substitute for up to one year of experience) either a) Providing direct services to persons with serious mental illness, developmental disabilities, alcohol or substance abuse; OR b) Linking persons who have serious mental illness, developmental disabilities, alcohol or substance abuse to a broad range of services essential to successfully living in a community setting.

- Appearance is professional and complies with CCOR policy.
- Ability to lift up to 25 pounds and to sit, stand, bend, stoop, kneel, pull, climb, reach, and perform repetitive movements of the upper extremities.
- Pass a physical examination and immunization requirements after being offered employment and complete a health assessment annually, thereafter.
- Possess a current, valid New York State driver’s license and a reliable vehicle.
- Completion of CCOR's orientation program.
