Responsibilities:
- Provide supervisory support to Housing Case Managers and oversee program operations in the absence of the Program Coordinator. Assist with assigning cases to staff.
- Manage a caseload of four clients and provide housing case management services for Nassau and Suffolk County clients. Assess clients’ needs and provide advocacy/referral services that address unmet needs.
- Maintain confidential client records assuring that client progress in meeting objectives are documented in a timely and fully detailed manner.
- Help residents with social interaction in their environment, diffusing conflicts that may arise with medication, support and service referrals.
- Serve as primary contact to address client and landlord related concerns. Assist all Housing Case Managers with apartment searches and ensure rents are within the allotted budget. Conduct initial housing unit inspection for all new units.
- Review all new leases and track expiration dates of current leases. Coordinate with staff and landlord all changes that need to be implemented, as well as negotiating the terms of the lease.
- Network with Continuum of Care (CoC) housing service providers, government agencies, parish outreach/ministry programs, and other human and community service agencies, providing program outreach and developing community awareness of the program. Attend regular meetings and events.
Qualifications:
- Bachelor’s Degree in a Human Services discipline.
- 5 years of experience working with homelessness and mentally ill populations.
- 3 years of supervisory experience.
- Regular travel to program sites and home visits in Nassau and Suffolk Counties. Must have transportation.
- May have to work flexible hours to address after-hours client emergencies/crises.
Schedule: Full time - 35 hours per week.
Rate of Pay: $45,500 - $49,140 annual
This rate of pay serves as a good faith estimate. Actual pay is based on a number of factors which may include a candidate’s qualifications, skills, competencies, experience and work location. The salary range or rate listed does not include any differential pay or other forms of compensation, such as the value of benefits, which may be applicable to this position.
