Position Overview:
The ARC Care Coordinator is a core member of the ARC Treatment Team who is responsible for supporting the coordination, engagement and follow-up of clinically indicated services for individuals and families who initiate services at the Crook County ARC. The ARC Care Coordinator acts in a supportive role to the ARC Manager and collaborates on services and follow up with the ARC Peer Support Specialist, supporting clients and families in navigating necessary services and support. The ARC Care Coordinator regularly consults with individuals and their families to determine their needs, educate them about their conditions and options, coordinate services, provide referrals, and conduct warm handoffs.
ARC functions and Services
•Screening and Assessment: Welcome youth and families, complete initial screenings, and refer and assist in coordinating additional assessments as needed.
•Brief Intervention: Provide short-term and extended brief interventions for youth who misuse substances, including family involvement when appropriate.
•Referral to Treatment/Care Coordination: Provide care coordination, follow up for at least 120 days, and coordination with support services to address barriers like transportation and communication.
•Peer/Family Support Services: Ensure continuity of care through coordinating ongoing support from peer support specialists.
•Community Education/Outreach: Create a stigma-free environment for learning about behavioral health and community services, and host regular informational groups as assigned.
ESSENTIAL POSITION RESPONSIBILITIES:
1. Support the therapeutic environment: Ensure appropriate care, treatment, safety, and stabilization in collaboration with ARC treatment team members.
2. Implement evidence-based practices: Model and implement practices that create a positive culture.
3. Coordinate care: Work with ARC staff and other stakeholders such as families, probation officers, school staff and other mental health providers as needed.
4. Screen and assess: Conduct screenings and assessments for substance use disorders and provide education about treatment options and referrals to community resources.
5. Participate in intake and admissions: Complete admissions processes, including verification of benefits, intake paperwork.
6. Collaborate with ARC staff: Refer to ARC manager if mental health assessment is indicated. Collaborate with ARC Peer Support Specialist to coordinate and implement brief interventions and support services.
7. Help individuals and families deescalate crisis and achieve improved mental health and behavioral stability.
8. Develop and maintain relationships: Build strong collaborative relationships with community partners.
9. Facilitate educational workshops: Lead or co-lead workshops, group skills counseling, and parent education groups.
10. Assess family understanding: Evaluate families' understanding of their child's problems and clarify expectations for evaluation and treatment services.
11. Coordinate referrals: Work with ARC staff to refer clients to alternative levels of care.
12. Support medication management: Assist with psychotropic medication management, focusing on treatment adherence, side effects, and effectiveness.
13. Provide brief behavioral interventions: Use evidence-based techniques such as CPS, motivational interviewing, or other treatments as appropriate.
14. Develop and implement care plans: Collaborate with counselors to create care plans addressing personal needs and overcoming barriers to sustained recovery.
15. Assist with resource access: Help clients and families access resources like social security, food, housing, and physical healthcare.
16. Participate in caseload consultation: Engage in regular consultations with the client's treatment team and communicate treatment recommendations.
17. Document patient progress: Record patient progress and treatment recommendations in EHR and other required systems.
18. Facilitate referrals and warm handoffs: Coordinate referrals and warm handoffs for services outside the organization.
19. Monitor external referrals: Oversee referrals to external substance use treatment services and facilitate access to intake appointments.
20. Attend training and meetings: Participate in mandatory training and meetings.
21. Perform other duties as assigned.
Requirements
QUALIFICATIONS, KNOWLEDGE AND SKILLS:
1. At least 3 years' experience in care coordination or clinical practices.
2. CADC 1 or 2
3. Experience providing direct services, particularly related to coordination of care between healthcare and behavioral health and/or addictions services providers.
4. Must have a compassionate nature.
5. Ability to provide exceptional customer service.
6. Cultural competency, bicultural proficiency and a desire to work within a community health setting and on behalf of disenfranchised communities.
7. Excellent communication, organization, and time management skills.
8. Ability to develop strong collaborative relationships with a range of community partners.
9. Flexible and dependable work ethic.
10. A commitment to the stated mission and values of Rimrock Trails.
11. Ability to work effectively both individually and as a member of a team.
12. Ability to tolerate ambiguity and utilize excellent judgment in a rapidly changing environment.
13. Understands and accesses community resources to assist with referrals and disposition of patients, including the ability to seek insurance verification.
14. Competent in client risk assessment, safety plans, crisis intervention, behavior management, case management and care coordination.
15. Adept at performing screening, clinical assessment, and treatment of substance use disorders.
16. Knowledge of applicable Oregon Administrative Rules.
17. Demonstrated ability to collaborate and communicate effectively in a team setting.
18. Ability to maintain effective and professional relationships with clients and other members of the care team.
19. Working knowledge of differential diagnosis of common substance use disorders and experience with evidence-based treatments and brief behavioral interventions (e.g. motivational interviewing, collaborative problem-solving etc.)
20. Experience with screening, assessment and treatment planning for substance use disorders.
21. Ability to effectively engage clients in therapeutic relationships.
Preferred qualifications:
1. BA/BS in Behavioral Health, Social Work, or a related field.
