Full-Time Medical Social Worker – Hospice - North of Santa Fe, NM

Connected Health Care is partnering with a leading hospice organization in the North of Santa Fe, NM to hire a compassionate and dedicated Full-Time Medical Social Worker . This is an exciting opportunity to make a meaningful impact by supporting patients and families during their most vulnerable moments. Position Overview:
As a Hospice Medical Social Worker , you will play a key role in supporting patients and families through psychosocial assessments, counseling, and resource navigation. You'll collaborate closely with the interdisciplinary hospice team to ensure holistic, compassionate care is delivered.

Key Responsibilities:
Conduct psychosocial assessments to evaluate patient/family needs

Provide emotional support and counseling to patients and families

Assist families with planning for care, financial issues, community resources, and end-of-life decisions

Coordinate care with nurses, physicians, chaplains, and other team members

Advocate for the patient and family while respecting cultural, religious, and personal beliefs

Educate families about advance directives and available support services

Participate in case conferences and care planning meetings

Qualifications:
Education:
Master’s Degree in Social Work (MSW) from an accredited school by the Council on Social Work Education; or

Baccalaureate degree in Social Work from an institution accredited by the Council on Social Work Education; or

Baccalaureate degree in psychology, sociology, or other related field supervised by an MSW

Experience:
At least one (1) year of experience in a healthcare setting required

Experience in hospice or palliative care highly preferred

Strong communication and interpersonal skills

Empathy, emotional resilience, and ability to work with grieving families
