JOB TITLE: Children’s Single Point of Access (CSPOA) Specialist
REPORTS TO: Director of Children’s Services
DEPARTMENT: Children’s SPOA
HOURS: 35 per week; 9:00 am - 5:00 pm

QUALIFICATIONS:
Master’s Degree in relevant mental health field, with 2 years of experience in behavioral health or related human services field with youth and families.

JOB FUNCTION:
The RiseWell Community Services CSPOA Specialist is responsible for evaluating referrals to the Suffolk County CSPOA, a centralized referral and monitoring system for youth with serious emotional disturbance (SED) who need mental health services to remain at home or in their community. CSPOA can direct youth ages 5-21 and families to a variety of community services.

ESSENTIAL FUNCTIONS:

· Serving as the contact for high need youth in the community and a liaison to referral sources such as hospitals, schools and other community providers.

· Establishing and maintaining positive relationships and open communication with referred families, referral sources, governmental and community entities.

· Providing education and technical assistance to partner programs, families, community-based and governmental organizations.

· Processing incoming referrals, including inquiries from families and providers, and tracking progress through to service connection and monitoring outcome.

· Creating, maintaining, and updating a list of community resources; assisting with linkages to those services; and identifying specialty services available in the community for youth and families.

· Collaborating with and educating a wide array of local and community stakeholders, (e.g., hospitals, pediatricians, schools, etc.).

· Providing outreach and educational presentations to promote the CSPOA process and inform providers and the community of available resources.

· Traveling to local hospitals, schools, residential facilities, training sites, and/or other community locations. Travel outside the County may be required.

· Performing other duties as assigned.

REQUIREMENTS:

· Master’s Degree in relevant mental health field with 2 years of experience in behavioral health or related human services; LMHP preferred

· Proficiency in Word, Excel, Outlook, Adobe and PowerPoint and IT platforms.

· Excellent written and verbal communication and interpersonal skills.

· Strong organizational skills, attention to detail and time management.

Job Type: Full-time

Pay: $54,000.00 per year

Schedule:

  • 8 hour shift
  • Monday to Friday
