About Job

RiseWell Community Services is seeking a Full Time LPN – Primary Care Coordinator to join the CCHBC Program.

REPORTS TO: Program Director
DEPARTMENT: Clinical Services

In this role, you will provide clinic and community based medical assistance for mental health and substance use clients to achieve the best outcomes for their clients. Duties involve working with clients directly and often working with the clients’ family members and other health care providers to obtain health metrics required by CCBHC guidelines at regular intervals and on an as need basis.

QUALIFICATIONS:
Currently licensed as an LPN by the New York State Education Department. Minimum 1 year experience in an outpatient treatment or rehabilitation setting.A valid and clean NY State driver license. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
