We are currently seeking a Licensed Social Worker (LBSW or LMSW) to join our outstanding care team and help ensure our residents and families receive the highest level of psycho-social support.

Position Summary:
The Social Worker is responsible for providing counseling, support, and resource coordination to residents and their families, while ensuring compliance with state and federal regulations. This role is a vital part of our interdisciplinary team, contributing to residents' overall well-being and quality of life.

Key Responsibilities:

• Conduct psycho-social assessments for new and existing residents

• Assist with admission, discharge, and transition planning

• Advocate for resident rights and coordinate necessary services

• Provide emotional support and counseling to residents and families

• Participate in care planning meetings and collaborate with nursing and therapy teams

• Maintain documentation in compliance with regulatory standards

Qualifications:

• Current Texas LBSW or LMSW license required

• Long-term care or skilled nursing facility experience preferred

• Knowledge of Medicaid/Medicare and discharge planning procedures

• Strong interpersonal, communication, and advocacy skills

• Empathetic, organized, and solution-oriented
