Mission:

The mission of Philip’s Academy Charter School of Paterson (PACS) is, we will provide a moral and rigorous education to a diverse student body, accomplished in a hands-on, technology infused educational environment that embraces differentiated instruction and individual attention that will allow our students to attend college, while developing a commitment to global citizenship, environmental sustainability, and personal virtue.

Nature and Scope of Job: Under the supervision of the Director of Student Special Services, the Case Manager will perform the duties necessary to meet the case management needs of students in a school setting. Philip’s staff participate in extensive professional development, collaborate with other faculty and staff, promote rigorous learning opportunities, and look out for the collective and individual interests of Philip’s students as well as the schools’ greater community.

Job Functions and Responsibilities:

  • Review thoroughly the Individualized Education Program (IEP) for each student assigned to caseload.
  • Conduct individual and group counseling for elementary school students.
  • Support students with social/emotional needs.
  • Connect families to community resources.
  • Provide education around trauma-informed practices.
  • Ensure that appropriate services, accommodations, goals, and behavior intervention plans for each student are provided and are being implemented.
  • Ensure all special education services are in place including secondary and related services.
  • Contact necessary secondary and related service providers to be sure services are provided.
  • Coordinate the appropriate distribution of classroom accommodations, supplementary aids and services, and behavior intervention plans with regular education teachers/administrators/bus drivers.
  • Ensure formal Notice of IEP/Eligibility Meeting is sent to parents prior to the meeting. Ensure appropriate efforts and contact has been made regarding annual reviews, addendums, and eligibility meetings.
  • Data Collection for Annual Goals: Continually monitor progress on each IEP goal and schedule IEP meetings when necessary to reevaluate goals and services. The case manager should reconvene the IEP team if progress is inconsistent or not being made.
  • Coordinate statewide testing accommodations for all students with disabilities with a testing coordinator.
  • Ensure all Annual Review IEP meetings are held prior to the meeting date of the last annual review meeting.
  • Ensure formal Notice of IEP/Eligibility Meeting is sent to parents prior to the meeting. Ensure appropriate efforts and contact has been made regarding annual reviews, addendums, and eligibility meetings.
  • Ensure progress reports are completed every 9 weeks and sent home to parents.
  • Ensure data collection/progress monitoring is being completed for IEP goals.
  • Ensure the implementation of the Individual with Disabilities Education Act of 2004 (IDEA) and its regulations and New Jersey Special Education State rules.
  • Follow and advocate the rules of confidentiality.
  • Attend special education department meetings.

Qualifications:

  • Master’s degree in Social Work
  • Hold a valid NJ State School Social Worker Certification. License Social Worker (LSW) a plus but not required.
  • Communicate effectively in English, both orally and verbally. Bilingual preferred.
  • Knowledge of Social Work principles, theories, testing, methods, etc. as well as proven behavior management methods.
  • Knowledge of differentiated instruction based upon student learning styles.
  • Knowledge of data information systems, data analysis and the formulation of action plans.
  • Knowledge of applicable federal and state laws regarding education and students.
  • Commitment to ensuring academic success for all students.
  • Shared dedication to Philip’s Academy’s philosophy and mission.
  • Excellent teamwork, work ethic, and organizational skills.
  • Possess excellent integrity, a gentle spirit, and demonstrate strong moral character and initiative.
  • Demonstrate technological abilities: word processing, data management, and informational retrieval.
  • Submit a background check through the New Jersey Department of Education.
  • Be CPR, First Aid, and Blood Borne Pathogen certified or obtain certification upon hire.
  • Complete a Tuberculosis test and physical and provide a physician’s documentation of both.
  • Be able to kneel, bend, squat, lift, push, pull, and carry students up to 70lbs.
  • Have dependable transportation.
