Hospice Social Worker

PRN

Hours Will Vary (Visits can be performed on the weekend/paid per hour) Average is 5-10 hrs per week. Team meeting every other week on Wednesdays in person preferred but can be virtual, monthly staff meetings on thursdays (can be virtual) and visits/documentation, etc. This can be worked around a full-time position. Patient visits are in-person and can be performed after 5PM week days or weekends.

Evaluates patients/family's appropriateness for care and individual psychosocial needs. Develops plan of care in coordination with Interdisciplinary Team and provides social services as applicable through the performance of a variety of services, interventions and modalities. Cares for neonate, infant, child, adolescent, adult, and geriatric age clients. Performs clinical duties in accordance with population specific guidelines as outlined in policy and procedures, and applicable regulatory standards.

Georgia license in Social Work (LMSW or LCSW). Maintains current BCLS certification.

Master's degree in Social Work from a school accredited by the Council on Social Work Education required

1-3 years' experience in the hospice setting is REQUIRED!

Requires reliable transportation
