About Job

This position exemplifies service excellence by providing the full array of case management services with a trauma-informed and person-centered approach. The Team Lead role utilizes community partners and resources to expand an individual’s access and engagement to appropriate holistic behavioral healthcare. Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with accessing social determinants of health. Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, grants, and Lifepath Systems policies and procedures. Documentation which meets fidelity standards is expected. Working independently and collaboratively to foster better outcomes for individuals served and community partners, to facilitate improvement in measurable ways, is central to LifePath Systems mission to serve. Team Leads train new workforce members, conduct monthly coaching, observe team members quarterly, and complete regular chart audits. They lead by example through outstanding service provision, attitude, and documentation practices. Leads facilitate team meetings and cover caseloads as needed. Reliable transportation and valid Texas Driver License required. Other duties may be assigned to meet the needs of the program and individuals served.

Education, Training and Experience (including licensure and certification)
  • Must have a Bachelor’s Degree from an accredited college or university with major course work in a human
services field as required to qualify as a Qualified Mental Health Professional – Community Services (QMHPCS) and two (2) years prior experience in a mental health field preferred.
  • Master’s Degree in human services field preferred.
  • English/Spanish bilingual preferred.
  • Experience providing tele-mental health services/coaching highly desired
  • Experience in crisis response and criminal justice system strongly preferred
Knowledge, Skills and Abilities
  • Applicable knowledge of community resources, of case management principles, objectives, standards, and
methods; and of program policies and procedures.
  • Be able to complete detailed, yet concise progress notes which meet fidelity standards on all services provided
during the sessions.
  • Ability to assess individuals’ needs, to coordinate services, to communicate effectively and to train others.
  • Have knowledge of the social services system and the ability to connect individual with appropriate services.
  • Excellent knowledge in tele-platforms, including exceptional ability to engage via multiple tele platforms and
guide individuals in tele set-up.
  • Knowledge and implementation of a strengths-based person-centered approach.
  • Ability to write clearly and professionally on all services and internal communication.
  • Ability to think clearly and logically to navigate potentially dangerous and/or emotional situations.
  • Ability to respond to individuals with patience, empathy and understanding.
  • Good time management, organization, attention to detail, and computer proficiency are integral.
  • Demonstrates strong multi-cultural competency.
  • Bilingual (English/Spanish) preferred
