Reports to the Epworth Clinical Operations Manager. Responsible for collaborating with treatment team to coordinate a successful discharge plan for assigned patients.

Serves as resource to treatment team by:
- Maintaining current listing of available community resources.
- Assisting with pre-admission screenings; assembling admission packets for state hospital referrals.
- Assisting Clinical Social Workers with organizing court hearings and associated legal paperwork.
- Documenting all discharge planning activities in the medical record.
- Attending care plan meetings.
- Gathering information to meet the needs of the patient (and/or family).
- Prioritizing referrals as they relate to current workload and discharge timeframes.
In conjunction with the treatment team, develops discharge plan to address individualized patient needs by:
- Consulting with Clinical Social Workers / Clinical Therapists and Attending Physician regarding individualized patient needs.
- Identifying patient and family needs for service and referring to community agencies and resources to address the needs and/or problems.
- Making patient transportation arrangements at the time of discharge.
Makes referrals as recommended by the treatment team by:
- Coordinate placement in group homes, residential treatment facilities, local shelters, nursing homes, etc. as needed
- Referring patients to Social Security, Medicaid, and other financial resources.
- Assisting with accessing outpatient services, including but not limited to, medication, outpatient therapy, and psychiatry appointments for patients at discharge.
Maintains professional knowledge, skills, and competence in areas of responsibility by:
- Staying abreast of recent developments in field of expertise.
- Attending in-services and department meetings; also participating in continuing education and attending conferences as approved.
- Developing and maintaining effective working relationships with community agencies/facilities, groups, etc. to be familiar with available resources.
Contributes to the overall effectiveness and efficiency of the Department by:
- Participating in Hospital activities and committees when appropriate.
- Serving as a resource to the Hospital and the community in area(s) of expertise.
- Completing other job-related assignments and special projects as directed.

- Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
- Adheres to regulatory agency requirements, survey process and compliance.
- Complies with established organization and department policies.
- Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience
- The above level of knowledge, skills and abilities are normally acquired through the successful completion of a bachelor’s degree in social services or a related field. Discharge planning or case management experience is preferred.

- Demonstrates ability to develop and maintain effective and positive working relationships and the skills necessary to effectively communicate and collaborate with treatment team. Also requires ability to gain trust and cooperation.
- Demonstrates the ability to maintain professional composure under pressure and the ability to handle crises effectively.
- Demonstrates the interpersonal and communication (both verbal and written) skills necessary to communicate in a tactful, sincere and effective manner with patients from a variety of backgrounds, healthcare professionals, various Hospital staff and agencies outside the Hospital.
- Demonstrates proficiency in basic computer skills (i.e., data entry, word processing and related applications); also requires ability to access information on the Internet.
- Demonstrates the ability to maintain flexibility and a positive attitude.
- Demonstrates and maintains ethical conduct and the ability to exercise discretion. Also requires a solid understanding of the Hospital policies/practices related to patient confidentiality.

Working Conditions
- Works in a high-stress office and patient care environment with rapidly shifting priorities and competing demands. Also has some contact with patients and family members who may be under considerable stress.
- May be exposed to biohazards.
- Ability to work flexible hours to handle urgent situations.
- Works in a patient care environment.
Physical Demands
- Requires the physical ability, stamina, visual acuity, and manual dexterity (i.e. to stand/walk for prolonged periods of time, push wheelchairs/carts, etc.) to perform the essential functions of the position.
