Job Title: Intake Coordinator

Job Summary:
L & G HOME CARE AGENCY is seeking a detail-oriented and compassionate Intake Coordinator to join our team. This role serves as the first point of contact for new clients and referral sources, ensuring a smooth and efficient onboarding process for home care services. The Intake Coordinator is responsible for gathering client information, verifying eligibility, coordinating assessments, and facilitating seamless communication between clients, families, and internal teams.
Key Responsibilities:
Client Intake & Assessment:
- Handle incoming referrals via phone, email, and fax from hospitals, doctors, social workers, and other referral sources.
- Collect and document necessary client information, including medical history, insurance, and service needs.
- Coordinate initial home assessments with the nursing and care coordination teams.
Documentation & Compliance:
- Ensure all required documentation is completed accurately and complies with state and agency regulations.
- Verify insurance eligibility and authorization requirements for services.
- Maintain organized records of intake files in the agency’s system.
Communication & Coordination:
- Act as a liaison between clients, families, caregivers, and other departments to ensure smooth service initiation.
- Provide information about home care services, agency policies, and available resources.
- Follow up with clients and referral sources to ensure timely service start dates.
Administrative Support:
- Update and maintain intake tracking logs and databases.
- Assist with scheduling initial home visits and coordinating with field staff.
- Support office staff with additional administrative tasks as needed.
Qualifications & Skills:
Education & Experience:
- High school diploma or equivalent (Associate’s or Bachelor’s degree in healthcare administration or a related field is a plus).
- Experience in home care, healthcare intake, or customer service is preferred.
Skills:
- Strong communication and interpersonal skills.
- Ability to multitask and prioritize in a fast-paced environment.
- Proficiency in Microsoft Office (Word, Excel) and electronic medical record (EMR) systems.
- Knowledge of Medicaid, Medicare, and private insurance policies related to home care services is a plus.
