Piper Shores is Maine’s only lifecare community, offering Independent Living, Assisted Living, Memory Support and Skilled Nursing services.

Due to internal advancement within the organization, we are looking for a full-time, 40 hour per week Social Worker/Admissions Coordinator to develop, plan and deliver services within the department designed to meet the physical, spiritual, and emotional needs of residents and their families. This role functions as the Licensed Social Worker for our 40-bed skilled nursing home, which is a CMS 5-star rated and CARF accredited facility with private rooms, excellent staffing ratios, and an average of 4-5 short-term admissions per month.

ESSENTIAL JOB DUTIES:

  • Maintains relationships with referral sources and Piper Shores Community Health Services, performing pre-admission clinical and financial reviews with leadership staff, and the orientation and registration of all new admissions.
  • Maintains regular and on-going relationship with families and residents to discuss needs or concerns, mediates issues that may arise between resident/family/staff.
  • Maintains appropriate case mix/census
  • Provides care coordination and discharge planning services through the IDT, utilization review, and transition team process.
  • Ensures safe discharge to the general community or transition to the appropriate level of care within the Piper Shores community.
  • Provides Social Work services and completes all required Social Service assessments and documentation in the Nursing Facility.
  • Functions as the public relations person (liaison to the community and health care agencies), providing tours of the facility and pertinent information regarding admissions.
  • Ensures compliance with regulatory standards.
  • Conducts educational sessions for staff.
  • Facilitates coordination of resident and family concerns.
  • Completes all mandatory documentation in a timely fashion as established by applicable regulations.
  • Participates in MDS 3.0 process and care planning for all residents as it relates to psychosocial well-being.
  • Participates in Quality Assurance activities.
  • Participates in memory care program development work group meetings to advance services to our residents.
  • Maintains the self-respect, personal dignity, and physical safety of each resident.

QUALIFICATIONS:

  • Current Licensed Social Worker in Maine, or eligible for reciprocity.
  • Minimum 2 years of experience in long-term care or other health care setting working with the elderly preferred. Public relations, marketing, or admissions experience in a health care organization preferred.
  • Understanding of a positive approach to care as well as person-centered approach preferable.
  • Working knowledge of state and federal regulations for long term care preferred.
  • Excellent interpersonal, verbal and written communication skills.
  • Ability to manage multiple priorities.
  • Knowledge of MS Office Suite, along with ability and desire to learn software programs that would enhance productivity.
  • Valid driver’s license.
  • Ability and willingness to attend educational sessions both on-site and off-site. This may include occasional overnight travel.
