Jewish HomeLife is looking for a Social Worker to join our Skilled Nursing division, The William Breman Jewish Home!!!

Summary: The long-term care social worker reports to the Director of Social Work Services. They are responsible for providing social work services to residents, their families and responsible party. They are responsible for social work, resident care planning and related documentation. When necessary, the social worker will assist with the responsibilities and duties of the Director of Social Work Services.

Qualifications:

  • Bachelor’s degree in Social Work (BSW), required.
  • Master's degree in Social Work (MSW), preferred.
  • Knowledge of working with the elderly, preferred.
  • Knowledge of Jewish values and customs, preferred.
  • Proficient computer skills.

Responsibilities:

  • Performs all long-term care resident MDS assessments.
  • Schedules and participates in long-term care resident care plan meetings.
  • Attends to psychosocial needs of long-term care residents and their families.
  • Document and update Code Status for Long-term patients – Do Not Resuscitate Order or Full-Code.
  • Calls families/responsible parties regarding residents' needs.
  • Mediates between resident/staff and resident/resident.
  • Attends interdisciplinary update meetings, as needed.
  • Attends all federally mandated staff meetings.
  • Participates in monthly and quarterly Quality Assurance meetings, as needed.
  • Facilitate/coordinate medical, dental, eye, podiatry and other services for residents.
  • Facilitates enrollment in partner programs for long-term care residents including Optum.
  • Completes all required documentation for long-term care residents including updating electronic medical record with care plan notes, assessments, and documenting patient and family interactions.
  • Facilitates and coordinates all required forms and paperwork for Medicare compliance for long-term care residents.
  • Cross train daily responsibilities with Director of Social Work Services.
  • Facilitates new move-ins' to long-term care, provide resources and support to new families.
  • Provide programming consistent with JHL core values.
  • Other duties as delegated by supervisor.

Supervisory Requirements: None.

Physical Requirements: Ability to lift up to 30 pounds; ability to sit for 80% of the day; ability to stand for long periods of time as needed

Mental Requirements: Ability to focus for extensive period of time on data, letter writing and other administrative duties. Requires mental acuity and ability to interact with residents, family and staff.

Job Type: Full-time

Pay: $51,500.00 - $55,000.00 per year
