Phoenix Center for Rehabilitation and Pediatrics is seeking a compassionate and organized Social Worker to join our interdisciplinary team. Our facility provides care to both pediatric and adult residents with complex medical needs, and we’re looking for someone who can support our residents and families with empathy, professionalism, and strong communication.

Responsibilities:

• Conduct psychosocial assessments and develop individualized care plans

• Provide emotional support and counseling to residents and families

• Assist with discharge planning and coordination of post-discharge services

• Advocate for resident needs and ensure their rights are protected

• Collaborate with nursing, therapy, and administrative staff to promote holistic care

• Maintain accurate and timely documentation

Requirements:

• Bachelor’s degree in Social Work or related field (MSW preferred)

• Current NJ CSW or LSW license required

• Prior experience in a skilled nursing or healthcare setting strongly preferred

• Understanding of Medicaid, Medicare, and insurance-related processes

• Strong interpersonal, organizational, and problem-solving skills

• Ability to work with both pediatric and adult populations
