The Reentry Program prevents individuals recently released from Illinois Department of Corrections (IDOC) facilities from experiencing homelessness when returning to their communities. Through a partnership with the Center for Housing and Health, reentry households receive subsidies for monthly rent, financial assistance for moving fees and other prohibitive costs, such as furniture and transportation, and intensive wrap-around supportive services with Trilogy.

The Reentry Program will provide permanent supportive housing for individuals recently released from an Illinois prison or jail and returning to Chicago. The program prioritizes individuals with chronic illnesses, including moderate to severe mental illness, Murphy Class members (individuals with sex offense convictions), and other individuals considered “hard to house” and otherwise unable to leave an IDOC facility without a housing placement.

The Housing Case Manager (HCM) will assess the client’s personal, medical, emotional, social, and environmental situation to plan for treatment course through home, community, and office visits. They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance use/addiction disorders utilizing a harm reduction approach. The HCM will provide at least 75% of all community support services in the community.


Responsibilities:

  • Coordinate the establishment of an individual recovery plan and ongoing care with the client, client’s support system, and other care providers (ie parole, court mandated services, CHH, medical providers etc)
  • Complete daily progress notes based on services provided to clients in a timely manner reflected in Trilogy’s documentation policy, the project guidelines, and agency standards
  • Complete all necessary documentation related to client care in Trilogy’s Electronic Medical Record (Carelogic) and the project system (Caseworthy).
  • Provide referrals and support in linkage to additional services (food pantries, employment, therapy etc) as indicated.
  • Assist clients in identifying signs and symptoms of de-compensation, assess for crisis situations and/or the need for stabilization through hospitalization. Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced.
  • Independent assessment of risk and client’s need for higher level of care; i.e. hospital, detox program, emergency room, etc.
  • Determine associated risk and coordinate support from colleagues/Supervisors to manage risk; e.g. provide two-person visits to clients
  • Assist clients in learning and improving independent living skills; i.e. personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items
  • Educate and assist clients with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK
  • Support clients with transition off master leases and accessing additional pathways to permanent housing based on eligibility (CHA, SRN, HUD PSH, senior housing, etc)
  • Accompany clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, physical and mental health, and medication.
  • Support with the development of partnerships with re-entry organizations in Chicago to best serve unique client needs.
  • Advocate on client’s behalf and empower clients to advocate on behalf of themselves when appropriate
  • Meet all CHH grant scopes, deliverables and timelines as required.
  • Participate in monthly System Integration Team meetings with CHH, IDOC, behavioral health team members, and any other partner Trilogy/CHH may identify
  • Communicate effectively within the team model for a multi-disciplinary approach to client care with a shared caseload.
  • Perform other related duties and/or projects as assigned.

Qualifications:

  • Must have one of the following requirements:
    • High School Diploma or GED and two years of supervised clinical experience in a mental health setting.
    • Associate’s or Bachelor’s degree in any field.
  • Experience working with people with mental health and/or substance abuse diagnoses preferred
  • Valid Illinois Driver’s License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
  • Strong risk assessment skills; and ability to independently make decisions
  • Ability to adapt and be flexible with changing work environment
  • Experience in community-based services preferred
