Must have hospital acute care case management experience and acute care discharge experience Unit Information: SOCIAL WORKER (917) Facility: HCA FLORIDA LAWNWOOD HOSPITAL Address: 1700 S 23rd Street, Fort Pierce, FL 34954 Floor: Building: Cost Center: 917 General Information Tell us about the unit(s): Number of Beds: 344 beds at acute care hospital (excluding BH and acute rehab) Patient Ratios: SW if pair with Matrix of another RN will work Complex case of 8 or 9. If they are just covering DC planning then will have 1:25 or up to 30 Documentation System: MIDAS/Meditech, Nate, Navihealth, Care Team Assignment Call Requirements: All work every third weekend Floating policy: All Social workers float-except trauma. Patient Experience requirements: Knowledge of Hospital DC planning/execution and knowledge of MDR or IDT Typical Hiring Profile Must have: MIDAS/Meditech Experience, PASRR, Navihealth MSW or LCSW List Typical Procedures Performed on Unit(s): Discharge planning, MDR, Referrals, Working with Family and Patients Additional Preferences: (ex: previous travel experience preferred, easily adaptable to fast paced unit, can function well independently) N/A Dress Code: n/a Awards/Recognition: Feel free to include items such as links to videos that feature the department, outstanding department/team awards, etc. n/a Show more
