Summary:
This position is primarily responsible for coordinating home and community based services to individuals and families within agency policies, coordinates social programming for the property, and provides social service intervention as needed by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Core duties and responsibilities include the following. Other duties may be assigned.
Assess residents who may require assistance in daily living activities or who require a higher level of care or who are in transition to a higher level of care. Involve family and other professionals, as appropriate.
Makes referrals to community service agencies and follow-up on service.
Coordinates with hospital staff regarding discharge planning for residents returning to their apartments.
Provides crisis intervention with residents on a short-term basis, as needed.
Provides case management services for residents receiving services from various community agencies.
Advocates for residents.
Maintains current knowledge base of services and programs impacting seniors and/or disabled (or resident population being served) to enhance ability to assist residents in securing assistance.
Identifies education and health programs to enhance the residents quality of life.
Establishes linkages and maintains contacts with community agencies offering services to residents.
Develops and up-dates resource directory.
Builds an informal support system for residents with family members, other residents, friends, etc. as needed.
Publishes and distributes a building newsletter.
Fundraises in order to cover the cost of programs and activities in the building.
Demonstrates sensitivity and responsiveness to cultural differences within the resident population.
Maintains resident confidentiality.
Utilizes social services software to maintain resident files in compliance with HUD requirements.
Reports all instances where there may be a question of abuse or neglect and take proactive steps to protect residents.
Participates in overall planning for activities at the property.
Serves as a resource to the Manager related to on-going policy development and social planning within the building.
Communicates and coordinates with property management staff regarding program and resident issues.
Participates in professional organizations and training opportunities in order to meet HUD requirements for CEUs. Secure the needed continuing education credits to meet HUD requirements and attend HUD training as necessary.

Must be punctual and timely in meeting all requirements of performance, including, but not limited to, attendance standards and work deadlines; beginning and ending assignments on time; and scheduled work breaks; where applicable.

QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE:
High school diploma or general education degree (GED); and a minimum of two years related experience and/or training; or equivalent combination of education and experience.
LANGUAGE SKILLS:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
COMPUTER SKILLS:
To perform this job successfully, an individual should have knowledge of: Spreadsheet Software (Excel); Word Processing Software (Word); Electronic Mail Software (Outlook); and Presentation software (PowerPoint).
CERTIFICATES, LICENSES, REGISTRATIONS:
- Education (Desired): Bachelors Degree or Associates Degree in Social Work, Counseling, Sociology or other social science major.
- MSW or Masters in Counseling
- Community organization
OTHER SKILL, ABILITIES, AND QUALIFICATIONS:
- Ability to effectively communicate with residents (handle concerns, complaints and issues); community organizations and providers; and other staff.
- Ability to interface and network with community organizations and provider agencies.
- Must be able to work in a fast-paced, multi-faceted and customer service-oriented environment.

- Social Service software experience
- Education (Desired): Bachelors Degree or Associates Degree in Social Work, Counseling, Sociology or other social science major.
- Previous experience as a Social Service Coordinator
- Access to reliable transportation
