About Job

The Palliative Care Coordinator is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Palliative Care Coordinator is responsible for the patient chart, including establishing the chart upon the initial referral and the ongoing maintenance of documentation until the patient is discharged and the chart is closed in accordance with Medicare/Medicaid rules and regulations. S/he facilitates the flow of patient care from initial consultation to patient discharge, including patient scheduling and phone triage. Additionally, s/he is responsible for the submission and tracking of patient billing and maintenance of patient metrics. Position Specific Responsibilities include supporting agency and corporate policies, attending staff meetings and educational programs, communicating needs and solutions to supervisor, establishing patient charts, ensuring referral documentation is signed and dated, submitting requests for medical records, filing documents accurately, scheduling visits, confirming appointments, answering calls, rotating deceased/discharged patient charts, performing billing tasks, coordinating communications with Palliative Care service members, faxing documentation to physicians, and performing other duties as assigned. Education and experience required include a high school diploma or GED, with two to three years of related experience highly preferred, and experience as a Licensed Practical/Vocational Nurse preferred. Skills required include mathematical skills, language skills, and other abilities such as understanding and speaking English and embracing integrated healthcare at home philosophy. Physical demands include standing, walking, manual dexterity, lifting up to 25 pounds, and visual acuity.
