About Surve

Surve is South Florida's premier concierge home health and wellness provider, delivering purposeful, personalized care to seniors and individuals seeking elevated support for mind, body, and spirit. Our services span from skilled nursing and home care to immersive wellness programming — all grounded in a high-touch, concierge experience.

We’re now launching our mental wellness division, and we’re looking for a Licensed Clinical Social Worker (LCSW) who can provide outstanding care and lead the development of our emotional and cognitive support programs. If you're passionate about therapy, innovation, and building something meaningful, we want to talk.

About the Role

We’re hiring a Lead Therapist (LCSW) to serve as the face and clinical foundation of our emotional wellness offerings. You’ll work directly with clients to provide one-on-one therapy and group sessions, while also helping build the clinical standards, workflows, and strategic roadmap for our mental wellness division.

This is an entrepreneurial role — ideal for someone who wants to practice clinically and help shape a premium, forward-thinking mental health service.

Responsibilities

  • Provide individual therapy to private-pay and Medicare clients, with a focus on seniors and aging-related issues (grief, life transitions, anxiety, cognitive decline)
  • Lead group therapy sessions focused on emotional health, mindfulness, processing, and cognitive stimulation
  • Help design and scale Surve’s mental wellness offerings, including wellness workshops, support groups, and custom programs
  • Maintain compliance with Medicare documentation and billing requirements (you must be Medicare-eligible)
  • Collaborate with our Director of Nursing and clinical team to align mental and physical care plans
  • Serve as the clinical point of contact for mental health services within the organization
  • Participate in client care planning, assessments, and progress reviews
  • Eventually help hire, train, and supervise future therapists as the department grows

Qualifications

  • Licensed Clinical Social Worker (LCSW) in the state of Florida (required)
  • Eligible to bill Medicare for outpatient therapy services (required)
  • 5+ years experience providing therapy to adults/seniors
  • Group therapy experience strongly preferred
  • Comfortable working independently and helping build systems from the ground up
  • Strong interpersonal, documentation, and care coordination skills
  • Concierge, private-pay, or wellness industry experience a plus

What We Offer

  • Flexible schedule with autonomy to shape your caseload
  • Premium clientele and a high-end wellness model
  • Administrative support for scheduling, billing, and coordination
  • Opportunity to grow into a department head or clinical director role as the company expands
  • Work within a mission-driven, wellness-forward organization that truly values mental health
