Friend Health is seeking a compassionate and experienced Care Manager - LPN to join our team. In this role, you will be responsible for care coordination and patient support in alignment with managed care and Patient-Centered Medical Home (PCMH) principles. The goal of this position is to enhance patient care quality and ensure continuity of services, working collaboratively with the healthcare team to improve health outcomes.

Essential Job Duties & Responsibilities:
- Develop, implement, and evaluate care management policies, goals, and objectives
- Support the care team in care coordination and implementation of patient care plans
- Conduct ongoing clinical assessments and develop integrated patient care plans for high- and medium-risk patients
- Collaborate in the selection, development, and maintenance of patient education materials
- Develop individualized care plans based on patient needs
- Work closely with the supervisor to integrate care coordination services into the team model
- Monitor daily activities of assigned staff, assess performance, provide feedback, and develop training and professional growth plans
- Provide orientation, education, and training for staff as needed
- Assist with transition of care post-hospitalization
- Participate in integrated care team rounds
- Perform data analysis and tracking
- Investigate and resolve patient and provider complaints
- Participate in relevant departmental and organizational committees
- Perform other related duties and projects as assigned
Educational Requirements:
- Current State of Illinois license (LPN, LCSW, or LSW)
- Diploma or Certificate from an Accredited LPN Program (Required)
- Current CPR Certification (Required)
- Valid Illinois Driver's License (Required)
- Case Management or Care Coordination Certification (Preferred but not required)
Experience & Related Skills:
- 3 years of experience in healthcare care coordination or disease management
- Experience in an ambulatory healthcare setting or federally qualified health center (FQHC) is required
- Strong time management and organizational skills in a high-intensity, patient-focused work environment
- Demonstrates problem-solving, decision-making, and sound judgment
- Exceptional customer service skills
- Strong oral and written communication skills
