About Us: A Better Solution In Home Care is a leading provider of home care services, committed to delivering compassionate and high-quality care to our clients. We are looking for a dedicated and experienced Field Care Coordinator to join our team and contribute to the well-being of our clients.
Position Summary: As a Client Care Coordinator, you will be responsible for a range of client care coordination activities, including client assessments, care planning, supervising field staff, and quality assurance. You will play a critical role in ensuring that our clients receive the best possible care tailored to their needs.
Key Responsibilities:
- Meet with clients and families to complete inquiries/intakes as needed.
- Perform client assessments to determine appropriate service mix, personal care needs, and the need for outside services.
- Coordinate and liaise with outside service providers, ensuring safety issues and concerns are documented and addressed.
- Open new cases, complete service agreements, and input client care plans into the Generations program.
- Conduct and document quality assurance visits with clients.
- Walk caregivers into cases to introduce them to clients/families/facilities and ensure they understand the care plan.
- Provide direct client care as needed/directed.
- Perform transportation services with clients as needed.
- Conduct outreach activities and discuss ABS services with potential clients and referral sources both over the phone and in person.
- Actively participate in marketing and staff meetings, contributing ideas to marketing strategies.
- Collaborate with the Staffing Department to ensure effective staffing of cases in assigned territories.
Qualifications:
- Minimum one year of experience in a homecare/home health agency or assisted living facility and skilled nursing facility
- Understanding of the intake, admissions, and assessment process in a healthcare environment.
- Experience as a caregiver/CNA or understanding of proper caregiving practices and techniques.
- Strong interpersonal skills and the ability to communicate professionally with clients, families, caregivers, and community professionals.
- Excellent telephone etiquette and the ability to discuss sensitive issues with clients and families.
- Strong organizational skills, attention to detail, and the ability to work independently.
- Reliable transportation and the ability to commute throughout the service territory.
- Proficiency in Microsoft Office (Word, Excel, Outlook) and the ability to learn additional software programs as needed.
- Bilingual skills are a plus
Job Type: Full-time
Pay: $22.00 - $26.00 per hour
Expected hours: 40 per week
Work Location: This position offers a flexible hybrid schedule, with a mix of work-from-home responsibilities and regular field visits within your assigned territory.
