Position Details:
- Job Title: Care Coordinator
- Pay: $23.00 - $25.00 per hour
- Caseload: 50–100 clients per month
- Job Type: Full-time | Contract
- Work Location: Seattle, WA. Field-based – meeting clients where they are
- Schedule: Monday to Friday, 10:00 AM – 6:00 PM
Position Overview:
The Care Coordinator plays a key role in promoting client health and well-being by providing comprehensive care coordination, quality assurance, and client advocacy. This position focuses on building strong relationships with clients and their families to ensure they receive timely, appropriate care and access to essential resources.
We are seeking a compassionate, organized, and adaptable professional who can thrive in a fast-paced, client-centered environment. The ideal candidate will be tech-savvy, highly responsive to client needs, and passionate about supporting vulnerable populations across King and Pierce Counties.
Key Responsibilities:
- Conduct outreach to new clients and schedule visits to introduce Health Home benefits.
- Complete comprehensive health assessments in collaboration with interdisciplinary teams and primary care providers.
- Develop and regularly update individualized Health Action Plans (HAP) based on client goals and evolving needs.
- Coordinate and connect clients to appropriate health care, mental health, housing, crisis intervention, and social support services.
- Advocate for clients and assist them in navigating complex health care systems and community resources.
- Provide educational materials and promote client and family engagement in care planning.
- Support clients with entitlement program applications and connect them to local resources and community events.
- Track post-hospitalization care and follow-up after major medical events to ensure care continuity.
- Promote health literacy and empower clients to actively participate in their care.
Qualifications:
Required:
- Bachelor’s or Master’s degree in social work, psychology, human services, behavioral sciences, or related fields.
OR
- Current licensure as a Registered Nurse (RN), Advanced Registered Nurse Practitioner (ARNP), Licensed Practical Nurse (LPN), Psychiatric Nurse, Psychiatrist, Physician Assistant, Clinical Psychologist, Mental Health Counselor, Agency-Affiliated Counselor, Marriage and Family Therapist, or Certified Chemical Dependency Professional.
OR
- Certified Medical Assistant with an Associate Degree, or Indian Health Service (IHS) Certified Community Health Representative (CHR).
Additional Requirements:
- Must be at least 21 years old.
- Ability to travel throughout King and/or Pierce Counties for in-person visits.
- Reliable vehicle, valid Washington State Driver’s License, and current automobile insurance required.
- Must pass fingerprint background check and other applicable state/federal screenings.
- Mileage reimbursement is included in the hourly rate.
Preferred Skills & Competencies:
- Experience in clinical settings or direct patient care (Required).
- Strong customer service and communication skills.
- Excellent time management and organizational abilities.
- Ability to work independently and collaboratively.
- Proficient in computer systems and willing to learn new software platforms.
- Knowledge of HIPAA regulations and commitment to client confidentiality.
- Fluency in English required; bilingual applicants are strongly encouraged to apply.
