Description:

We are seeking a compassionate and collaborative Care Manager to join our Medical Respite team. In this role, you will work closely with both internal team members and external providers to ensure seamless, patient-centered care for individuals recovering from illness or injury while experiencing homelessness.

The Care Manager will provide clinically appropriate case management throughout each guest’s stay, supporting their recovery and helping to coordinate services aligned with their care plan. This includes fostering independence, maintaining therapeutic boundaries, and encouraging active engagement in their medical treatment.

As a key member of the interdisciplinary care team, you’ll help ensure that all aspects of each individual’s care plan are carried out effectively and appropriately, with the goal of promoting stability, wellness, and successful transition beyond respite care.

Requirements

Master’s Degree in Human Services and 1 year of experience OR a Bachelor's degree in Human Services and 3 years experience.

Experience and/or interest in working with the homeless, at risk and individuals with substance use disorder.

Pre-employment drug testing

Must be able to obtain ACT 33/34 and FBI clearances

SCHEDULE

Tuesday thru Saturday 8am-4pm OR 9am-5pm, with flexibility to meet persons served care plans and coordinate late admissions
