Position Summary:
We are seeking a highly organized and compassionate Hospice Intake Coordinator to join our team. This role is crucial in facilitating seamless hospice admissions, verifying insurance, and guiding families through the intake process with empathy and clarity. The ideal candidate will be well-versed in hospice services, comfortable with patient and family communication, and capable of working collaboratively across departments.

Key Responsibilities:

• Coordinate all aspects of the hospice admission process, ensuring timely and accurate completion
• Conduct insurance verifications and authorizations
• Serve as the initial point of contact for patients, families, and referral sources
• Educate families and caregivers on hospice services, philosophy, and care expectations
• Collaborate with clinical, administrative, and outreach teams to support a smooth transition to hospice care
• Maintain accurate and detailed documentation within EMR systems
• Participate in interdisciplinary team meetings as needed

Qualifications:

• Previous experience in hospice or healthcare intake preferred
• Understanding of hospice philosophy, admission criteria, and Medicare/Medicaid guidelines
• Excellent communication and interpersonal skills, especially in emotionally sensitive situations
• Ability to multitask and stay organized in a fast-paced environment
• Proficient in electronic medical records and basic office software

Pay: From $22.50 per hour
