Position Summary
As a Medical Social Worker, you will play a vital role in supporting patients and their families during challenging times. You will provide short-term counseling and crisis management to patients facing significant adjustments due to illness, hospitalization, or lifestyle changes. Your responsibilities will include:

  • Conducting psycho-social assessments and developing discharge plans.
  • Collaborating with healthcare professionals, patients, and families to ensure a smooth transition out of the hospital.
  • Identifying community resources and guiding patients through complex healthcare systems.
  • Documenting all assessments and strategies in our electronic health record systems.

Position Requirements
To qualify for this position, you must hold a Master's degree in social work or a closely related field and be licensed by the Tennessee Health Related Boards. An LCSW is preferred. Previous experience in a hospital or clinical setting is essential.

Performance Standards
You will be expected to:

  • Provide prompt psychosocial assessments and discharge planning within 24 hours.
  • Document assessments accurately and maintain communication with the healthcare team.
  • Screen patients for additional discharge needs and coordinate post-discharge referrals.
  • Maintain positive relationships with community agencies and provide timely information.
