About the Position:
We are seeking a compassionate and motivated Home Health Social Worker to join our interdisciplinary care team. The ideal candidate will hold a Master's degree in Social Work (MSW), be licensed in the state of Alabama (LMSW or LICSW), and demonstrate a passion for supporting individuals and families navigating health challenges in the comfort of their own homes.
As a Home Health Social Worker, you will provide emotional support, community resource linkage, and care coordination to home-bound clients, often during vulnerable and transitional periods. You'll work closely with nurses, therapists, and physicians to ensure holistic, person-centered care.
Responsibilities:
Conduct psychosocial assessments and develop individualized care plans
Provide counseling to patients and families regarding adjustment to illness, caregiving, and health care related concerns
Coordinate with community resources for financial, housing, mental health, and social service support
Support care transitions (e.g., long-term care placement, rehab placement, palliative care and hospice referrals)
Document timely and thoroughly in compliance with regulatory and agency standards
Collaborate with interdisciplinary team to promote patient safety, dignity, and well-being
Qualifications:
Master's Degree in Social Work (MSW) from an accredited institution
Active LMSW or LICSW license in Alabama- must show proof of licensure
Minimum of 1 year experience in home health, hospice, medical social work, or similar setting preferred
Strong knowledge of local resources
Strong interpersonal, organizational, and documentation skills
Valid driver's license and reliable transportation for home visits
