General Statement

The Social Work Intern, under general supervision, performs professional level casework by completing assessments to determine individual's eligibility in the Senior Nutrition program. Work involves receiving referrals via phone, email or voicemail. Documenting the referral in the ServTracker database, calling potential participants and completing phone interviews or home visits. Additionally, employees will complete timely reassessments to ensure that there are no gaps in services. Employees may make monthly contact via phone all with meal recipients as a measure to determine if meals are being received and evaluate if there are any changes in participant's health or living conditions. Employees analyze situations and determine appropriate courses of action immediately or in short time spans and often under very stressful conditions. Requires current enrollment in a bachelor's degree program.

This is an unpaid internship.

Essential Functions

Performs in-home and telephone assessments of SNP participants as required by state and program policy.

Maintains knowledge of governmental and private organizations and resources in the community to refer clients to in order to assist them in meeting financial, psychological, emergency care, emotional, or other needs.

Stays abreast of social work theory, techniques and practices, individual and group behavior, social problems, the aging process, medical and mental illness, family dynamics, coping behavior, crisis intervention, and treatment of various medical and mental diseases and social issues.

Routinely talks with clients identified through a screening process as high risk on the phone.

Answer phone calls from clients and/or their family members regarding inquires on meal service and/ or available resources.

Documents direct client interactions, services development, and referrals in accordance with agency requirements. Maintains case data and documentation through various computer programs.

Performs under occasional stress and may be confronted with persons acting under stress.

Follows all guidelines set forth, which include state and federal laws, program rules, directives, policies and regulations, professional ethics and quality assurance standards.

Serves as inter/multi-disciplinary team member.

Delivers meals to homebound participants as needed.

Performs other related duties as assigned.

Qualifications

Education and Experience

Requires current attendance in Bachelors of Social Work program with an accredited university; or a four-year degree in a human services field.

Licenses or Certifications

Valid Driver's License with a safe driving history.
