If you’re passionate about helping others, and eager to join a work family that values your unique drive and dedication, join us in setting a higher standard for in home senior care!Learn more about how we’re raising the bar in new ways in this quick 2-minute video at: .ROLE HIGHLIGHTS - What to Expect:As a Client Care Coordinator, you're the face of care, coordinating new Client intakes, onboardings, and facilitating excellent care plans to serve their needs well. You'll assist with business development and networking efforts. You’ll also provide outstanding customer service and careful documentation.
  • Travel regularly throughout Clatsop and Tillamook counties - building relationships and onboarding new Clients.
  • Network with community engagement organizations and referral sources, such as Case Managers, Discharge Planners, Senior Brokers, Rotary and the Chamber of Commerce.
  • Document, monitor, analyze, track, update, and report on Clients using CRM and care management system.
  • Intake new Clients during the initial in-home consultation.
  • Develop robust and comprehensive Care/Service Plans to provide customized care for each Client, according to their individual needs, preferences, and Payee scope of authorized/allowable services.
RequirementsROLE QUALIFICATIONS
  • Excellent verbal and written skills in English required.
  • Proficient in Excel, Microsoft Outlook, PowerPoint, and Word.
  • Excellent attention to detail and ability to maintain accurate records.
  • Thorough knowledge of best practices for handling PHI as governed by HIPAA
  • Experience handling highly confidential and sensitive information.
  • Outstanding attention to detail, project management, documentation, organizational, and planning skills.
  • Ability to take initiative and work independently.
  • Ability to work in a fast-paced team environment with a commitment to reach goals and meet deadlines.
  • Exceptional time management abilities to meet deadlines and manage multiple tasks simultaneously.
  • Outstanding positive attitude with the ability to grasp and embrace the company's overall goals and objectives and portray them to customers and staff. Able to work well with others in a collaborative and respectful manner.
  • A desire to be out in the community networking and building referral pipelines and assisting Clients
