Key Responsibilities:

  • Conduct Assessments: Evaluate clients' needs and develop individualized care plans.
  • Provide Counseling and Support: Offer emotional and psychological support through individual and group sessions.
  • Manage Cases: Coordinate care by working with clients, families, and other professionals; monitor progress and update plans as needed.
  • Advocate for Clients: Ensure clients have access to necessary resources and services; make referrals as needed.
  • Crisis Intervention: Respond to emergencies and provide immediate support.
  • Community Engagement: Conduct outreach and educational activities to promote social well-being.
  • Maintain Documentation: Keep detailed and confidential records of client interactions and progress.
  • Collaborate with Professionals: Work with healthcare providers, educators, and other social workers to deliver comprehensive support.

Qualifications:
Bachelor Degree
Master Degree
State SW licensure (LMSW, LCSW)
Professional Educator License (PEL)
