Position Overview: We are seeking a dedicated and compassionate Licensed Clinical
Social Worker (LCSW) to join our multidisciplinary team. This role involves providing high quality psychotherapy services alongside a Psychiatric Nurse Practitioner (NP) and managing chronic care coordination to enhance patient outcomes. The LCSW will play a crucial role in delivering comprehensive mental health support and coordinating care for patients with chronic conditions. We are looking for a provider to cover Assisted Living facilities in Terre Haute. We go into 1 facility/day. The hiring process requires a shadow with one of our current providers in the Indianapolis area.
Key Responsibilities:
Psychotherapy:
- Provide individual therapy to clients, employing evidence-based practices and
therapeutic modalities.
- Conduct thorough assessments of patients' mental health, including diagnoses and
treatment planning.
- Develop and implement treatment plans in collaboration with patients, their
families, and the Psychiatric NP.
- Monitor patient progress and adjust treatment plans as necessary.
- Maintain accurate and confidential records of therapy sessions and treatment
outcomes.
Chronic Care Management:
- Work closely with the clinical team to develop and implement care plans for
patients with chronic conditions.
- Assist in the coordination of care between various providers, ensuring a holistic
approach to patient health.
- Facilitate patient education and self-management strategies for chronic illness.
- Address psychosocial factors affecting patients’ health and treatment adherence.
- Advocate for patients’ needs and assist in navigating healthcare services and
resources.
Collaboration:
- Collaborate with the Psychiatric NP to integrate mental health care with overall
patient treatment plans.
- Participate in case meetings and care coordination discussions to ensure
comprehensive patient management.
- Provide insights and recommendations based on psychosocial assessments and
observations.
Documentation and Compliance:
- Maintain accurate and timely documentation of all patient interactions and
treatment plans.
- Ensure compliance with all relevant regulations, ethical standards, and
organizational policies

Pay: $75,000.00 - $100,000.00 per year
