About Job

The Hospital Care Coordinator is a professional responsible for coordinating hospice and palliative care patients in the hospital setting. The Hospital Care Coordinator strives for communication excellence by bridging the gap between patients, caregivers, and all members of the care team.

This person builds a trusting relationship with patients and caregivers by understanding their critical needs and conveys that information to the care team. The Hospital Care Coordinator plays a vital role and is integral at every hand-off throughout the patient journey.

Responsibilities:

Perform all care coordination needs (including):

  • Assist with education of the community regarding hospice and palliative care and associated services

  • Learns and understands the roles of all members of the care team

  • Conduct initial intake and review health history with patients and family

  • Enter and update demographics, ensure patient consents are signed, and document time working in

    chart

  • Follow-up on all tasks such as; open orders for e-prescribe, medication refills, DME, laboratory,

    specialist referrals, follow patients across care transitions

  • Assist in entering medications into EMR, as directed

  • Obtain consent or authorization forms, as directed

  • Follow-up with hospital Case Manager/Discharge Planner to coordinate hand off from hospital

  • Assists in gathering and maintaining medical records

  • Notify care team members when visits are not complete

  • Must provide Graduate of a Medical Assistant certificate program (required).

  • Palliative Care/Hospice/Home Health experience preferred

  • Demonstrates the knowledge of hospice philosophy and services and is able to communicate and

    apply them accordingly

  • Expert customer service ability and a strong desire to perform as an integral part of the care team

  • Possesses an ability to maintain good interpersonal relationships with patients, families,

    co-workers, and other health team members

  • Current driver license and your own vehicle
