Faith Hospice has unique opportunities for a Social Worker (LLMSW or LMSW) who feels the call to serving those patients at end of life and the opportunity to work as an essential part of an interdisciplinary health care team who share this very rare and special gift.

At Faith Hospice, we recognize that working with patients and their loved ones at end of life is hard work and so incredibly rewarding! We have support systems in place through EAPs, open door managers, and the opportunity to attend (and be paid for attending!) monthly support meetings hosted by our Bereavement Manager, if desired.


What Will You Do?

There are 2 MSW opportunities currently open:

  • Hospice Admissions MSW:
    • This is a full-time position working three 10-hour weekday shifts (9am – 7pm) per week plus every other weekend. Salaried as a full-time position with full-time benefits.
    • Visits on weekdays are more focused on hospital informational sessions and admissions. Visits on the weekend include patient visits and/or admissions in the community and in hospitals.
  • Hospice MSW: this is a full-time schedule working Monday - Friday days; conduct thorough assessments of social and emotional factors/issues:
    • family dynamics
    • financial and environmental issues
    • bereavement needs
    • patient/family/caregiver goals and communication styles
    • develop and implement the care plan in conjunction with the interdisciplinary group (IDG).

Education and Experience

  • MSW licensure in Michigan (LMSW or LLMSW with intent to become fully licensed)
  • At least 1 year of medical social work experience is required
  • Hospice and/or hospital experience preferred
  • Passion for serving hospice patients and families
