Cadence Care Network has an opening for an Early Intervention Service Coordinator.
Responsibilities include:
· Provide support and consultation to referred families in a home-based setting.
· Take primary responsibility for the development and implementation of the individual Family Service Plan (IFSP), which is developed with the family and service provider.
· Coordinate all necessary services/treatment, referrals, assessments, and timelines.
· Work in collaboration with other community providers and provide referrals for other services the family may need.
Qualifications:
· Bachelors degree in social work or child development/family studies or two years related work experience with another degree.
· Ability to work independently and collaboratively.
· Have a valid drivers license.
Job Type: Full-time
Pay: $45,000.00 - $47,000.00 per year
