Job Description
The Lead Care Manager is a site-based, client-facing role that supports clients temporarily housed with Illumination Health + Home. This role focuses on engaging and enrolling clients, promoting ongoing participation in medical, behavioral, and social services, and serving as their primary point of contact.
Working closely with the Department Manager, the LCM ensures contract compliance and program outcomes. This is a non-clinical or clinical position requiring strong knowledge of social determinants of health, service barriers, and effective care coordination. The role supports clients in navigating healthcare systems and staying connected to needed services.
The pay range for this position is $25.00 - $28.00 per hour, depending on experience
Responsibilities
Client Care & Coordination
Deliver comprehensive, non-clinical or clinical care management by assessing needs, developing and updating care plans, coordinating services, setting weekly goals, and tracking progress.
Maintain accurate, SMART-formatted documentation in Electronic Health Records (EHR) and databases (MIS, Champ, HMIS, or health plan systems).
Monitor and document client engagement levels, progress, referrals, and key records (e.g., vitals, insurance, benefit letters).
Participate in weekly Interdisciplinary Care Team (ICT) meetings to evaluate progress and adjust strategies.
Connect clients with essential services including healthcare, nutrition, hygiene, transportation, housing, and care team members.
Provide crisis intervention and ensure confidentiality in all aspects of service delivery.
- Promote monthly health education and support client attendance at medical appointments when necessary.
- Track client schedules, forms, outreach efforts, and missed appointments.
- Maintain a caseload of 10-20 ECM members, as needed
Supervision & Collaboration
- Lead regular care team meetings and provide oversight to care activities performed by team members, volunteers, and interns.
Support and supervise volunteers as needed to ensure consistent, high-quality service delivery.
Participate in one-on-one supervision with management to discuss program updates and client needs.
Documentation, Data & Compliance
Ensure timely and compliant documentation of all client interactions in line with contractual and KPI requirements.
Conduct routine audits of case notes and files to ensure accuracy, completeness, and adherence to guidelines.
Track and report client outcomes and program data using Microsoft Suite and EHR systems.
Billing & Reimbursement
Perform regular billing reviews to ensure services are accurately reimbursed and in compliance with pay standards.
Help minimize audit risks, denials, and revenue loss by maintaining thorough and timely documentation.

Preferred Experience/Minimum Qualifications:
Required:
Bachelor’s degree in social services, Healthcare, or related field; or equivalent combination of training and experience.
Experience in homeless services, care management, and mental health support
Current valid CA driver's license
Experience in non-profit housing and/or housing for people with disabilities and chronic health conditions.
Must be familiar with VI-SPDAT (if applicable).
Preferred:
At least 2 years of experience working with at risk/unhoused individuals
Bilingual in English and Spanish
