Licensed Independent Social Worker - Indian Hills Clinic (Full-Time) 070225

LOCATION: Family Health Care of Siouxland – Indian Hills Clinic

POSITION: Licensed Independent Social Worker

GENERAL SUMMARY OF DUTIES:
Provides professional team-based care to patients and assists in the continuum of care and assists in educating patients on their patient centered medical home

SUPERVISOR: Clinic Manager

QUALIFICATIONS:

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strength and problems related to their diagnosis, illness, treatment and/or life situation
Formulate, develop and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions
Assist with billing for services performed and completing appropriate documentation in the patient’s medical record
Provide quality customer service internally and externally
Team player with outstanding interpersonal abilities
Independent thinker
Excellent organizational and oral/written communication skills
Strive in a fast-paced clinic environment
Word processing and computer experience
Clinical office setting and Electronic Health Record (EHR) experience preferred.
Masters Degree in Social Work
Valid state licensing and certification required
