Description

This position will be working per diem/on call for our MLK facility on weekdays, day shift.

As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Spends a majority of time in direct service activities within the Center, interaction with other team members, case management and assessment of current care plans and ongoing assistance to participants to address social issues.

Required Qualifications:

Master's Degree - Social Work.

Washington Clinical Independent Social Worker Associate License upon hire or,

Washington Clinical Independent Social Worker Associate Temporary License upon hire or,

Washington Clinical Independent Social Worker License upon hire or,

Washington Clinical Independent Social Worker Temporary License upon hire.

2 years - Social work with geriatric age groups from diverse ethnic cultures.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
