Overview
We are seeking a dedicated and compassionate Eligibility Worker to join our team. In this role, you will be responsible for assessing the eligibility of individuals for various programs and services related to mental health and behavioral health support. You will work closely with clients to ensure they receive the appropriate resources and care they need, while maintaining compliance with HIPAA regulations.

Responsibilities

Conduct thorough patient assessments to determine eligibility for services.
Facilitate group therapy sessions and provide individual counseling as needed.
Collaborate with clients in developing personalized care plans.
Maintain accurate case management records and documentation in accordance with HIPAA guidelines.
Provide support in home community care settings, ensuring clients have access to necessary resources.
Implement behavioral therapy techniques to assist clients in achieving their mental health goals.
Work closely with multidisciplinary teams to coordinate care and support for clients.
Monitor client progress and adjust treatment plans as necessary.

Qualifications

Experience in clinical counseling or mental health counseling is preferred.
Knowledge of patient assessment techniques and case management practices.
Familiarity with behavioral health principles and practices.
Strong communication skills, both verbal and written, with the ability to build rapport with clients.
Ability to work effectively in a team-oriented environment while also being self-motivated.
Understanding of HIPAA regulations and commitment to maintaining confidentiality.
Previous experience in group therapy or individual group counseling is a plus.
