Social Services Coordinator
Provide the opportunity for residents to age in place, despite declining health problems, poverty and emotional stress by linking them to the community and supportive services they need to continue living as independently as possible. Develop strong resident retention programs and identify abnormal physical, behavioral and unit conditions seeking solutions for these conditions.
Responsibilities:
- Conducts and assists in coordinating resident and family support groups including the development of programs for special needs residents;
- Greets residents, visitors, and staff courteously, promoting resident rights, dignity, and confidentiality standards;
- Responsible for maintaining communication and contact with family, while encouraging their participation in care planning;
- Responsible for identifying and seeking ways to support residents’ individual needs and preferences, customary routines, concerns and choices;
- Coordinates social services and participates with the management team in care planning and development;
- Attends and participates in training sessions, in-services, and staff meetings;
- Provides discharge planning services by working with the facility management team, resident, resident family and physician;
- Assisting residents in voicing and obtaining resolution to grievances

Requirements:
- Bachelor’s degree in social work, Psychology, or a human services field including but not limited to sociology, gerontology, special education, rehabilitation counseling, and psychology;
- Two years of supervised social work experience in a health care setting working directly with individuals.
- Functional literacy in English is required;
- Effective interpersonal and communications skills are required;
- Prior experience in Social Work, working with residents with Dementia, and Alzheimer's Disease is preferred.
