PRIMARY FUNCTION/GENERAL PURPOSE OF POSITION:

The Social Worker works to address needs of at risk and complicated patients. The Social Worker addresses psychosocial issues of referred patients and families through consultation with the Nurse Navigator and/or face to face interviews. The Social Worker will be dedicated to ensuring patients have timely access to comprehensive medical treatment, increasing patient compliance with recommended treatment options, mobilizing access to all needed services, and creating a seamless, patient-centered experience from screening to detection, diagnosis, treatment and beyond.

ESSENTIAL JOB FUNCTIONS:


  • Community resources for multi-generational psychosocial needs, including financial, housing, mental health, substance abuse, domestic violence and bereavement
  • Adult and Child Protective Services
  • Medicare & Medicaid policies and criteria
  • Experience navigating Medicaid, Medicare and Private insurance
  • Medical, legal and financial implications for patients
  • Advance directives and “Do Not Resuscitate” orders
  • Patient assistance programs for free or reduced cost medications
  • Psychological pain and addictions
  • Alternative methods for addressing pain (such as mindfulness, guided imagery, and relaxation techniques)
  • Working knowledge of the DSM 5 (or most recent DSM)
  • Grief & bereavement counseling and resources
  • Procedures for
  • Placement (Nursing, SNF, ALF, Private Home Care)
  • Home Health Referrals
  • Hospice Referrals
  • Mental Health & Substance Abuse Referrals
  • The Affordable Medical Care Act
  • Proficiency in basic computer skills, including the ability to maneuver within a Microsoft Windows environment.

REQUIRED QUALIFICATIONS:


  • Master’s Degree in Social Work
  • License Clinical Social Worker Certification
  • 2 years of experience in a medically related Social Work position.
  • Basic Life Support (BLS) – American Heart Association


PREFERRED QUALIFICATIONS:


  • Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
  • Certification by a nationally recognized association such as IBHRE is preferred.


Training:


  • EPIC Electronic Health Record (EHR) training (preferred)
