Salary Range: $37420.42 - $48960.27 / annually
Role Description
This is a full-time hybrid role for a Service Coordinator at Patient Care Coordination, Inc. The Service Coordinator will be responsible for daily coordination tasks to connect participants with appropriate community services. This role will be primarily located in Philadelphia, PA, with some work from home flexibility.
Qualifications
- Service Coordination and Operations Management skills
- Strong Interpersonal Skills and Communication abilities
- Customer Service experience
- Ability to work independently and collaboratively
- Experience in healthcare or social services field is a plus
- Bachelor's degree in Social Work, Psychology, Public Health, or related field
