Position Summary

As a Hospice LCSW, you will play a vital role in the care team, providing clinical psychosocial support to hospice and home health patients and their families. This role blends emotional care with practical assistance—supporting patients through advanced illness, guiding families through end-of-life decision-making, and helping all involved find comfort and clarity during a sensitive time.

Key Responsibilities

  • Conduct in-depth psychosocial assessments for hospice and home health patients.
  • Collaborate with the interdisciplinary team to create individualized care plans.
  • Offer clinical counseling and emotional support to patients and families.
  • Provide guidance on advance care planning, including directives, living wills, and POA documentation.
  • Assist families with navigating Medicaid/Medicare benefits, insurance, financial aid, and community resources.
  • Deliver grief and bereavement counseling, including anticipatory grief support.
  • Facilitate crisis intervention as needed during the course of care.
  • Maintain accurate, timely documentation in accordance with CNS and CMS regulations.
  • Participate in regular care conferences and provide team insight into psychosocial aspects of care.
  • Support families following the loss of a loved one through post-bereavement services.

Qualifications

  • Master’s Degree in Social Work (MSW) from an accredited university.
  • Current Utah licensure as a Licensed Clinical Social Worker (LCSW) required.
  • Minimum of 1 year of experience in hospice, home health, palliative care, or medical social work (preferred).
  • Deep knowledge of psychosocial issues in terminal illness, grief, and loss.
  • Skilled in patient/family counseling, therapeutic communication, and resource navigation.
  • Comfortable working independently in patients’ homes, with excellent organizational skills.
  • Valid driver’s license and reliable transportation for patient visits.
