- Shifts: 8:00 AM - 4:30 PM, 5 x 8 Days
- Weekend Requirement: Included in rotation (every 5th or 6th weekend); holiday coverage as needed
- No on-call or floating requirement
- Accepts RN Case Manager/Discharge Planner or Licensed Social Worker
- Social worker must be eligible for LMSW/LGSW/LCSW and have passed the exam
- DC RN Licensure: Will accept pending licensure with proof of application or hold LMSW/LGSW/LCSW
- BLS Certification
- 2 years of experience in discharge planning, including admission criteria review, benefits eligibility, care coordination, conducting needs assessments, and ordering DME
- Experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, and acute hospital experience
- Prior charge experience preferred
- Completion of at least 1 travel assignment
- Regulatory Skills: CMS, HIPPA, JCC; workers compensation preferred
- Unit Patient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU
- Charting: Cerner
- Uniform: Business casual, professional appearance required
- Flu vaccine required for all employees
