Position Details

• Shifts: 8:00 AM - 4:30 PM, 5 x 8 Days
• Weekend Requirement: Included in rotation (every 5th or 6th weekend); holiday coverage as needed
• No on-call or floating requirement
  
  

Requirements

• Accepts RN Case Manager/Discharge Planner or Licensed Social Worker
• Social worker must be eligible for LMSW/LGSW/LCSW and have passed the exam
• DC RN Licensure: Will accept pending licensure with proof of application or hold LMSW/LGSW/LCSW
• BLS Certification
• 2 years of experience in discharge planning, including admission criteria review, benefits eligibility, care coordination, conducting needs assessments, and ordering DME
• Experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, and acute hospital experience
• Prior charge experience preferred
• Completion of at least 1 travel assignment
• Regulatory Skills: CMS, HIPPA, JCC; workers compensation preferred
