POSITION OVERVIEW - Case Manager

Urban Triage Case Managers are cross-trained across all housing programs offered by Urban Triage and provide intensive, person-centered case management, system navigation, and service coordination. They center on addressing housing barriers, transformative education, and community outreach. Case Managers collaborate directly with clients, community agencies, and partners to promote housing stability through goal-setting and resources that address the client's barriers to housing, including employment readiness, mental health, financial empowerment, and community integration.

This position supports adults, youth, individuals, and families through comprehensive case planning, strength-based services, and strategic coordination with landlords, community agencies, and service providers. All services are delivered in alignment with Urban Triage’s Mission, Vision, and Values.

QUALIFICATIONS

  • Commitment to the mission and values of Urban Triage, Inc.
  • Experience in case management and non-profit experience or related social service work is preferred.
  • Strong interpersonal, communication, multitasking, and crisis intervention skills.
  • High organizational skills, time management, and attention to detail.
  • Intermediate computer literacy skills and general math (multiplication, percentage, division, etc.). ,
  • Must be flexible, self-motivated, and able to work independently and within a team.
  • Comfortable with community outreach and relationship building. Going into the community, self-led and motivated to support the work of Urban Triage.
  • Must pass a background check.
  • A valid driver’s license, insurance, and reliable transportation.
  • Being bilingual (English/Spanish) is a bonus.

TASKS & RESPONSIBILITIES - CASE MANAGEMENT

  • Conduct client walk-ins, intakes, and regular follow-up appointments.
  • Build report with clients through compassionate conversation before paperwork.
  • Identify key barriers to housing: income, credit, criminal history, rental background, participation, mental health, AODA, and desire to be housed.
  • Facilitate housing plans based on client-identified priorities and root cause barriers.
  • Develop 3 SMART goals per client, with three action steps and three resource referrals per goal, reviewed and updated at each meeting.
  • Create and maintain detailed case notes, service plans, and appropriate documentation in all databases and physical client files.
  • Conduct weekly meetings with clients in-office or in the community.
  • Comply and adhere to all case management processes and guidelines Urban Triage defines.
  • And other tasks assigned by Urban Triage

COMPLIANCE REQUIREMENT

Job Type: Part-time

Pay: Up to $20.00 per hour

Expected hours: No more than 40 per week
