Summary

We are seeking a compassionate and skilled Case Manager to join our team at CSC . In this role, you will play a vital part in supporting clients through their challenges, facilitating access to necessary resources, and promoting overall well-being. This position is crucial in helping individuals navigate their paths toward recovery and stability.

Responsibilities

Conduct assessments to determine clients' needs and develop personalized care plans.
Provide crisis intervention and support during critical situations.
Facilitate access to community resources, including mental health services and housing assistance.
Mentor clients through their recovery journey, offering guidance and motivation.
Supervise case management interns or staff as needed, ensuring quality service delivery.
Maintain accurate documentation of client progress and case notes in compliance with agency policies.

Requirements

Bachelor's degree in social work, psychology, or a related field; Master's degree preferred.
Experience in behavioral health or social services is highly desirable.
Strong crisis management skills with the ability to handle high-pressure situations effectively.
Excellent communication and interpersonal skills for building rapport with clients.
Familiarity with motivational interviewing techniques is a plus.
